Payment Policies

We file all insurance plans but ultimately SRI may not be credentialed or in network with your specific plan. Please contact your insurance provider with any questions about your plan, covered services, and in-network/ out of network providers.

The following forms can be downloaded to your computer and printed at home.

• Records Release Form to Alternate Facility

It is necessary that you arrive 30 minutes prior to your appointment to check-in and complete all new patient forms. You must present insurance and identification card upon check-in.

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Acrobat® Reader'. This will take you to Adobe's website and enable you to download the Acrobat program to your computer.
